Volleyball Camp Waiver & Release of Liability

Assumption of Risk

I understand that participation in volleyball camps, training, conditioning, games, and related activities involves inherent risks, including falls, collisions, strains, sprains, fractures, heat-related illness, and other injuries. I voluntarily authorize the participant to take part and accept those risks.

Medical Authorization

If I cannot be reached in an emergency, I authorize club staff to obtain reasonable emergency medical care for the participant. I understand that I am responsible for resulting medical costs.

Release and Hold Harmless

To the fullest extent permitted by applicable law, I release and hold harmless the organizing volleyball club, its directors, coaches, employees, volunteers, and participating facilities from claims arising from ordinary risks of participation. This provision does not waive rights that cannot legally be waived.

Insurance and Health Information

I confirm that the participant is physically able to participate and that I have disclosed relevant medical conditions, allergies, medications, and accommodations. I accept responsibility for maintaining appropriate medical insurance.

Conduct and Policies

The participant will follow coach instructions, safety rules, facility rules, and standards of respectful conduct. I have reviewed and accept the camp cancellation and refund policies.

Media Choice

Unless I notify the club in writing that I decline, I authorize reasonable use of camp photographs or video for club communications and promotion.

Parent or Guardian Consent

For a participant under 18, I certify that I am the parent or legal guardian and have authority to sign this agreement.
